The link to the voluntary Wisconsin School Health Services Survey is now open. You will find the link on the DPI School Nursing Data Collection webpage. As explained on the webpage, the report is voluntary, but the information collected is critical to understanding how health services are delivered in Wisconsin.
Private or charter schools are invited to submit data if it is not already part of data reported by your local school district. No personally identifiable student information is collected. Please submit data by August 15, 2021. When you read the questions, substitute “private school” for “school district.”
The data points for the 2021-2021 survey have been tailored in accordance with the demands of this pandemic school year. See the 2020-2021 data collection template for the data points collected by the survey.
